The internal sense of gender and the social expression of that identity. This axis integrates neurobiological influences (Axis 2) with devopmental, psychological, cultural, and social factors. Gender identity and gender expression are related but distinct dimensions of human experience.

Gender expression: Diverse forms of expression influenced by personality, culture, and social context. Gender expression may vary from day to day, or with movement within various cultural or social settings.
Why It Matters
Understanding the distinction between gender identity and gender expression helps explain why people with the same identity may express themselves differently. Expression is influenced by personality, culture, age, life experience, and social environment, while gender identity reflects an individual’s internal sense of self developed during gestational development. Recognizing this distinction reduces stereotypes and helps explain the diversity found within both transgender and cisgender populations.

Richards et al. (2016) — Non-Binary or Genderqueer Genders.
Reviewed the scientific and clinical literature on nonbinary and genderqueer identities, concluding that these identities are valid, stable, and recognized across diverse cultures and historical periods. The authors emphasized that nonbinary identities represent natural human variation rather than confusion or indecision.

Provides professional guidelines recognizing that gender identity and gender expression are distinct constructs. The guidelines emphasize that gender expression varies across individuals and cultures and should not be used to invalidate a person’s gender identity.
